Now, more than ever before, people need to be actively engaged in their personal health and health care. The countless lifestyle decisions we make on a daily basis have a dramatic impact on optimally preventing and managing chronic conditions. Yet changing these daily habits and behaviors is difficult!

Integrating lifestyle management issues within a standard medical visit poses considerable challenges. Yet even in optimal situations with clear understandings of recommendations from health providers, easy access to abundant health educational material, and a high degrees of motivation, changing behaviors and habits remains a challenge. Sustaining those behaviors consistently over time is even more demanding.

PosTrack Technologies has developed an individualized, collaborative solution we call HoIP (also called Health over Internet Protocol). HoIP is designed to activate, monitor, and support healthy behavioral changes in populations at risk for, or diagnosed with, chronic conditions such as obesity, diabetes, and cardiovascular disease. It is built on a solid foundation of behavior change theory and evidence-based recommendations for wellness and health management, and is designed to effectively support health behavior change on an individualized level, yet scale to efficiently manage any size, number, and type of population.

Patient-Centered Approach with Individualized Profiles and Goals

  • Behavioral Goals: Personalized, incremental behavioral goals collaboratively set with subscribers can include:
    • increasing physical activity,
    • modifying dietary behaviors,
    • improving coping skills and addressing stress or depression,
    • adhering with medication regimens, or
    • smoking cessation.
  • Adherence with Disease Management Protocols: Brief, relevant surveys of symptoms, biometric values, and behaviors can be delivered to individuals. Values such as blood pressure, weight, and blood glucose can be monitored on a real-time basis.

  • Care Coordination: Care coordination schedules can be supported via automatic delivery of reminders and messages, allowing interactive confirmations and responses.

  • Leveraging Preferred Technology: Subscribers securely access the HoIP system via the technology they most prefer – traditional phone or cell phone via Interactive Voice Response, text messaging, Smartphone mobile application, or Internet web browser on PC or MAC - or any combination of these technologies or modalities. Since users can choose the communication technology that best suits their comfort level and lifestyle, as well as receive optimally timed prompts to interact with the system, consistent engagement and long term retention is improved.
     
  • Enrollment and Assessment: Initial assessments guide individuals towards setting key achievable, measurable behavioral goals that will positively impact their health and confidence. Recommendations from providers are also easily entered into user profiles.
     
  • Powerful Feedback: Immediate and personalized reinforcement is provided at opportune points, based upon subscriber’s individual profile and real-time monitoring data, via HoIP's proprietary software and robust algorithms. Dynamic messaging, graphical trends, and usable reports motivate and educate users to continue to make good health decisions and progressively update their incremental health behavioral goals.
     
  • Human Support: Parameters set around behaviors and biometric values trigger exception reporting to behaviorally and medically trained call center staff, who directly intervene at opportune points to empower subscribers to stay on track and avert unnecessary health care or emergency department utilization.
